Please contact us to schedule your fingerprinting appointment. You will need to bring the following:
- Driver's license and Social Security card
- Substitute Registration form (click link to download)
- Current Negative TB EXAM (valid for 4 years)
TB skin test locations and info
- $57.00 check or money order only--fingerprint fee
